Under normal circumstances, it lives on the normal secretions of the skin (especially the secretion of the sebaceous glands) without producing any clinical manifestations. Under optimum conditions however, this organism trans- forms itself into another form which grows like a fungus, and is called Malassezia furfur and produces a disease called ‘pityriasis versicolor’.
The conditions which have been noticed to bring about this transformation include proper amount of heat and humidity (summer and rainy season), deficiency of the immune system of the individual as when the individual is having a disease or taking some drugs which suppress the immunity, and also if certain special types of fats are being secreted by the sebaceous glands of the skin. Certain individuals therefore, are more prone to develop pityriasis versicolor and aggravations of the disease are frequent during the summer and rainy season or in certain disease states or on treatment with certain drugs.
The disease is as a rule asymptomatic and thus the lesions may not be noticed for quite some time. Moreover, the lesions are most frequently seen on the back which is generally not inspected by the individual himself/herself. The earliest lesions consist of small circular areas of superficial scaling which are slightly lighter in colour than the surrounding skin. Such areas progressively increase and merge with each other to produce larger areas, which may involve most of the back, the upper part of the chest and may spread to the upper arms also. Involvement of the face, the lower limbs and other parts of the body is extremely uncommon. The disease however, remains asymptomatic except for occasional episodes of a mild pricking sensation, especially on exposure to sunlight or heat.
Several topical anti-fungal medicines are effective against this organism, and the infection can be eradicated in two to four weeks but the lightening of the skin areas takes a further one to two months to return to the normal colour. The patient therefore, may not be able to perceive the effect of the treatment till the skin colour has returned to normal.
Since the causal organism is a normal commensal of the skin, the disease is likely to recur whenever the conditions favourable for its growth are available again. Systemic drugs have also been used in daily or intermittent courses to prevent the overgrowth of the causal organism. Thorough cleaning of the skin however, helps to minirnise the recurrences. Pityriasis versicolor is a mild disease which may not be treated at all except for cosmetic reasons.
